RELEASE AND WAIVER OF LIABILITY (Print and fill in the blanks.)

This form must be read, signed, dated, and returned to Dr. Carlie (Strong 468) before you are eligible to participate in any CJSociety-sponsored ride-along. The undersigned, being the age of 18 or above, desires to participate in the Missouri State University Criminal Justice Society’s police ride-along in

Participation in this ride-along is for the purpose of observation and will involve travel by motor vehicle. I assume any and all risks associated with the trip, including but not limited to, collisions and personal injury involving motor vehicles, all such risks being known and appreciated by me including all risks associated with riding with law enforcement officers. I understand that my participation in this ride-along is entirely voluntary on my part.  

The undersigned understands that his/her participation in this activity is not covered by University-provided insurance. Knowing the risks, and in consideration of being permitted to participate in the ride-along, I for myself and my personal representatives, heirs and assigns, do hereby hold harmless and release, waive, discharge and covenant not to sue the Board of Governors of Missouri State University, Missouri State University, its Board members, officers and employees, including any member or sponsor of the Missouri State University Criminal Justice Society (hereinafter collectively referred to as "MSU") from any and all claims or liability on account of death or injury to person or property of the undersigned of any kind or nature whatsoever arising out of, or in any way connected with, the undersigned's participation in this ride-along, even though the claim or liability may arise out of the negligence or carelessness on the part of MSU or any third person, whether foreseen or unforeseen, known or unknown.

The undersigned hereby expressly agrees that this release and waiver is intended to be as broad and inclusive as permitted by the laws of the State of Missouri and that if any portion hereof is held invalid, it is agreed that the balance, notwithstanding, continue in full legal force and effect.

The undersigned further states that he/she has carefully read the foregoing Release and Waiver of Liability, knows the contents thereof, and has agreed to sign this Release and Waiver of Liability as his/her own free act and deed. Participant agrees that he/she is subject to and will follow University rules, including no use or possession of alcoholic beverages or illegal drugs.
